NHS Hospitalist Program Description
The goal of the NHS Hospitalist program is to provide a patient/family-centered model of quality care from admission to discharge using a multidiscipline approach.
Program Description:
· Inpatient program support to:
- Unassigned Medical,
- “ER admit no bed”,
- Appropriate transfers from other services,
- High risk post discharge assessment clinic as required
· Each Hospitalist will provide care for approximately 18-20 in-patients a day.
· Hospitalist will rotate on call; the goal is one in five weekday call, and one weekend in five.
· The high risk post discharge patients will be seen by the Hospitalist in the “Hospitalist Clinic” anywhere from 2- 3 days a week depending on the volume.
- Some Family Physicians continue to provide care for their own patients. The NHS Hospitalist program will support this decision and there may continue to be physicians who volunteer for on-call for the Hospitalist program.
- Extended Class Nurse Practitioner will participate in the Program and Clinic
Practice:
- Considered “independent contractors” practicing the profession of medicine.
- Hospitalist will enter a detailed contractual Professional Services Agreement for their services.
